Instructions
File with the school's Title IX Coordinator or directly with the U.S. Department of Education Office for Civil Rights. Describe the discriminatory conduct, including dates, locations, and individuals involved. Identify the school and explain how it receives federal funding. The school must respond promptly and equitably under its grievance procedures. You may also file with OCR within 180 days of the last act of discrimination. Retaliation against complainants is prohibited.
