Formal report of bullying, cyberbullying, or harassment of a student, triggering the school's obligation to investigate and take corrective action.
Instructions
Describe the bullying behavior in detail including dates, times, locations, and witnesses. Identify the alleged perpetrator and the victim. All 50 states have anti-bullying laws requiring schools to have policies and procedures. The school must investigate promptly and take appropriate action. If the bullying is based on a protected characteristic (race, sex, disability), it may also constitute illegal harassment requiring response under federal civil rights laws.
