Creditors must file by the bar date set by the court. State the amount of the claim, the basis for the claim (contract, loan, credit card, etc.), and whether it is secured, unsecured priority, or general unsecured. Attach supporting documentation (promissory note, invoices, account statements). Government claims typically have a later bar date. If you disagree with how your claim is listed on the debtor's schedules, you must file a proof of claim to preserve your rights.
