Pollution Incident / Spill Report
Report of a release or spill of hazardous substances, oil, or pollutants to the National Response Center as required by CERCLA, CWA, and EPCRA.
Instructions
Report immediately to the National Response Center (1-800-424-8802) for releases of hazardous substances exceeding reportable quantities. Provide the location, substance released, estimated quantity, time of release, and media affected (air, water, soil). Also notify state and local emergency response authorities. Follow up with a written report within the timeframes specified by the applicable regulation. Failure to report is a criminal offense.
