Form W-9 Request for Taxpayer Identification Number
Form used to provide a taxpayer identification number to a person or entity that is required to file an information return (such as a 1099).
Instructions
Provide your name (and business name if different), federal tax classification (individual, C corp, S corp, partnership, LLC, trust), address, and taxpayer identification number (SSN or EIN). Certify under penalties of perjury that the information is correct and that you are not subject to backup withholding. Return the completed form to the requester — do not send to the IRS. Never post your W-9 publicly as it contains your SSN or EIN.
