Instructions

Section 1 must be completed by the employee on or before the first day of employment. Section 2 must be completed by the employer within three business days of the start date. The employee must present original documents from List A (identity and work authorization) or one from List B (identity) and one from List C (work authorization). Retain I-9 forms for three years after hire or one year after termination, whichever is later.
