Instructions
Identify the student by name and student ID. Specify the records to be released (transcripts, disciplinary records, financial aid information). Name the recipient and the purpose of the disclosure. The consent must be signed and dated by the student (if 18 or older) or parent. Consent may be revoked in writing at any time. Schools may disclose directory information without consent unless the student has opted out.
