Employee Written Warning / Disciplinary Notice
Written documentation of employee misconduct or performance issues, specifying the violation, expected improvement, and consequences of continued problems.
Instructions
Describe the specific behavior or performance issue with dates and examples. Reference the policy or expectation that was violated. State what improvement is expected and the timeframe for improvement. Describe the consequences if the issue is not corrected (further discipline, up to and including termination). Have the employee sign acknowledging receipt. If the employee refuses to sign, note that on the form and have a witness sign.
