Advance Directive (Living Will & Healthcare Proxy)
Legal document specifying a person's wishes for medical treatment if they become unable to communicate and designating a healthcare agent.
Instructions
Complete both the living will section (specifying treatment preferences) and the healthcare proxy section (naming an agent). Address life-sustaining treatment, artificial nutrition and hydration, pain management, and organ donation. Sign in the presence of two witnesses who are not healthcare providers or beneficiaries. Some states require notarization. Provide copies to your physician, healthcare agent, and family members.
