Alleged violations include failure to call the 24-hour hotline, failure to submit 5-day written report, effluent exceedances for pH, 5-day biochemical oxygen demand, fecal coliform, and total suspended solids, missing information from the quality assurance project plan, missing information from the operation and maintenance plan, and failure to update the operation and maintenance plan annually.
