Case summary
Automatic line leak detectors were not tested for 2 years, the overfill prevention device had not been inspected at least once every 3 years, and the tightness test of the spill buckets had not been done within the last 3 years. During testing of the diesel line, the valve cracked while trying to get the valve free. The diesel product lines were manifolded. A new company took over operations of the facility and installed new product lines, submersible turbine pumps, under dispenser containment, spill buckets and overfill devices.
