Ag Link, Inc. did not (1) maintain up-to-date safety information related to the maximum intended inventory for aqueous ammonia or the codes and standards used to design, build, and operate the aqueous and anhydrous ammonia storage processes; (2) update the hazard review at least once every five years; (3) provide refresher training every three years to three employees operating the processes; (4) replace past due pressure relief devices mounted on the anhydrous ammonia storage tank; (5) certify compliance audits are conducted at least every three years; and (6) update their Risk Management Plan's emergency contact information and submit corrected information within thirty-days of the change. These six requirements help prevent an accident by including safety precautions and maintenance, monitoring, and employee training measures. The company agreed to pay a penalty of $4,920.
