Respondent had other permit violations such as failure implement spill prevention & emergency clean-up best management practices; exceedances of maximum pH hold time; late submittal of quarterly Discharge Monitoring Reports; failure to document, record, and/or maintain monthly visual inspection reports; and failure to submit annual reports. The Respondent agreed to pay a penalty of $82,000.00.
