Case summary
On February 4, 2009, the Oregon Department of Agriculture (ODA) conducted a producer establishment inspection of Formulation Packaging Specialties (FPS). The ODA inspection report was sent to EPA Region 10 where it was reviewed for violations. Our review found the company was using outdated labeling on six products they produced. The label deficiencies included missing directions for use, missing first aid statements, and missing storage and disposal statements. This SSURO contains the specific violations on each label and outlines what needs to be done to lift the SSURO.
