Case summary
In June of 2005, WSDA conducted a routine producer establishment inspection at CH2O, Incorporated in Olympia, Washington. Upon review of the inspection report, current product labels were requested from the company. When these labels were reviewed, six of them were not in compliance with the most recent EPA approved labels. Five of these labels only had minor errors and are being addressed in a warning letter to the company. The 6th product had significant violations in that the product label lacks critical statements pertaining to first aid, directions for use, and directions for disposal. For this product (PACT 5, EPA reg. no. 1448-171-43553) we are issuing a stop sale order
