Case summary
Sterigenics self-disclosed that for a period from at least 5/16/2006 until its discovery in March/April 2008, Sterigenics operated EO sterilization chambers with a control system switch turned from auto to manual . Failing to operate on auto caused otherwise preventable EO emissions to the environment during breakdowns of the DeOxx scrubber control device. Sterigenics fulfilled all audit policy requirements except D1 (systematic discovery) and therefore received 75% mitigation of the gravity part of the penalty. This self-disclosure also includes the adjacent Sterigenics facility at 4801 E 50th St.
