The violation involves failure to conduct and submit to EPA initial performance test data as required by section 62.14463(a); failure to submit to EPA values for the site-specific operating parameters for the operating incinerator as required by section 62.14463(b); failure to submit a waste management plan as required by section 62.14432; and failure to submit annual reports as required by section 62.14464(b).
