Case summary

The Order addressed several violations identified from September 2022 to present. Violations include failure to conduct a Level 1 assessment in September 2022; failure to revise an incomplete sample siting plan by March 2023; failure to conduct triggered source monitoring following a total coliform positive sample in September 2022; failure to monitor for total coliform bacteria in October 2022, January 2023, December 2023, March, May, and June 2024; failure to monitor for nitrate during 2023; and failure to notify the public of these violations.
