The inspection and follow-up investigation review determined that the facility failed to include the maximum intended inventory in the RMP, failed to establish a system to properly address the team?s findings and recommendations in a process hazard analysis, failed to develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process, failed to certify annually that the operating procedures are current and accurate, failed to provide refresher training at least every three years or more often if necessary, and failed to document results of inspections performed on process.
