Case summary

On August 31, 2016, the EPA conducted a stormwater construction inspection of the Frostwood F6 Townhomes construction site owned by Frostwood 6 LLC and operated by David Jacobsen Construction. Findings included, but were not limited to, failure to properly complete and certify a Notice of Intent to discharge under the Utah Construction General Permit, failure to prepare a complete Stormwater Pollution Prevention Plan (SWPPP), failure to provide a copy of the SWPPP to EPA inspectors in a timely manner, failure to ensure personnel conducting inspections were properly trained and certified, failure to document inspections and corrective actions, failure to properly manage concrete washout, and failure to properly install and maintain required stormwater and sediment controls. Additionally, discharges from the site have occurred since the permit coverage for the site expired on November 18, 2016.
