Case summary

Perma-Guard, Inc. a pesticide manufacturer headquartered in North Salt Lake City, UT, produced six EPA-registered pesticide products in 2014. On May 14, 2014 an EPA inspector conducted a routine FIFRA inspection. While preparing the inspection report the label review identified this facility had six misbranded labels. Additionally, this facility is a supplemental distributor of a product called Perma-Guard Crawling Insect Control, EPA Reg. No. 73729-1-67197. This product has not been registered with the EPA. On September 18, 2015, Region 8 issued a Stop Sale, Use and Removal Order (SSURO) to Perma-Guard, ordering the company to stop selling and distributing the misbranded products, to start using their current and approved labels, and to stop selling the unregistered pesticide.
