On the date of the Inspection, Respondent had failed to implement the SWPPP and to properly operate and maintain the BMPs described in Parts 3.l.D. and 3.6.A. of the Permit. As of the date of the Inspection, Respondent had not documented and retained stormwater self-inspections at the Facili ties as requi red by the Permit. Respondent's failures described above, constitute violations of Parts I.D., Part II.C., Part III.B. and Appendix I. A. of the Permit.
