During July and October 2012, two or more samples from the System were positive for total coliform, Respondent did not notify the EPA of the July or October 2012 MCL violations. After the System's water tested positive for total coliform on October 1, 2012, the Respondent failed to take at least five routine samples of the System's water in November 2012. Respondent failed to notify the public of the violations, Respondent failed to report the violation listed above to the EPA.
