RESPONDENT FAILED TO NOTIFY EPA OF THE PROBLEM. The Emergency Administrative Order issued responds to a situation which may pose an imminent and substantial health endangerment to persons served by the system, specifically, a possible line break before the system's storage tank on or about November 16, 2012. The situation caused service to be interrupted in the Skull Valley community. System was required to issue a boil order, provide alternate water supplies, restore service, disfect and flush, and begin daily monitoring for total coliform bacteria (followed by weekly, then monthly when advised by EPA). Also, system has 30 days to submit a plan/schedule to ensure no future pressure loss at system.
