Case summary
This notice of violation is issued to the Utah Department of Environmental Quality for violations at the Cedar Fort Municipal Center. These violations consist of failure to monitor for nitrate, nitrite; failure monitor for bacteriological quality; failure to collect set of repeat samples after a total coliform and/or fecal coliform positive result; failure to collect 5 additional routine samples the month following a total coliform positive result; failure to provide public notice of violation; and failure to report violations to the State. If the State does not initiate appropriate action within 30 days of the receipt of this notice, EPA will issue an Administrative Order.
