THE PCB WASTE WAS RECEIVED BY SAFETY KLEEN (GRASSY MOUNTAIN) A PCB DISPOSAL FACILITY IN UTAH. THE PCB WASTE WAS DISPOSED OF ON 5/15/1996, MORE THAN ONE YEAR AFTER IT WAS REMOVED FROM SERVICE FOR DISPOSAL CONSTITUTING A VIOLATION OF SECTION 15 OF TSCA.
