Case summary
Lighthouse for the blind was inspected July 5 & 6, 2023. During the inspection a notice of preliminary findings was left at the facility. Upon review it was determined the facility failed to make hazardous waste determinations, had open undated and unlabeled containers, had numerous satellite accumulation violations, failed to document weekly inspections, could not provide annual training documentation, failed to update the contingency plan, had unlabeled and open wipes containers, had unlabeled used oil container and had undated waste containers.
