​​Omaha Standard, LLC (�Respondent�) is the owner or operator at 3501 S. 11th Street Council Bluffs, Iowa 51501 (�Facility�). The EPA conducted a compliance evaluation inspection, on April 13-14, 2022. The EPA alleges that Respondent violated the following requirements of the RCRA hazardous waste management program. ​a. Failure to label universal waste lamp container was not labeled with the words �universal waste lamp(s),� or �waste lamp(s),� or �used lamp(s)�, 40 CFR 273.14(e) (NOPF No. 1). ​b. Failure to perform a weekly inspection of the CAAs as required by 40 CFR 262.16(b)(2)(iv) (NOPF No. 2). Not included in penalty. ​c. Failure to label hazardous waste accumulation container with an accumulation start date as required by 40 CFR 262.16(b)(6)(C) (NOPF No. 3). ​d. Failure to label hazardous waste accumulation container with the words �hazardous waste� as required by 40 CFR 262.16(b)(6)(A) (NOPF No. 4). ​e. Failure to label hazardous waste accumulation container with an indication of the nature of the hazard as required by 40 CFR 262.16(b)(6)(B) (NOPF No. 5). ​f. Failure to label hazardous waste accumulation container with all applicable waste codes as required by 40 CFR 262.11(g) (NOPF No. 6). Not included in penalty. ​g. Failure to close hazardous waste accumulation container as required by 40 CFR 262.16(b)(2)(iii)(A) (NOPF No. 7)​
