Recently, EPA was informed by the Arkansas Department of Health (ADH) that the Respondent failed to do the following: 1) collect and submit total coliform samples and submit monthly reports from October 2021 to present; 2) retain a certified operator; 3) address the Significant Deficiency of inadequate water operator license for the months of June 2021 to present; and 4) provide Public Notice of the Significant Deficiency (failure to retain a certified operator). Therefore, EPA is now ordering the Respondent to address the violations stated and provide public notification(s) for the violations stated and to submit a copy of the public notification(s) with an accompanied signed Certificate of Delivery to EPA and ADH through the issuance of this action.
