Recently, EPA was informed by the Arkansas Department of Health (ADH) that the Respondent failed to provide Public Notice for the following: 1) failure to submit valid bacteriological samples for the month of June 2018 and July 2020; and 2) failure to submit operations reports for the month of June 2017. Therefore, EPA is now ordering the Respondent to provide public notification(s) for the violations stated and to submit a copy of the public notification(s) with an accompanied signed Certificate of Delivery to EPA and ADH through the issuance of this action.
