Case summary
Recently, EPA was informed by the Arkansas Department of Health (ADH) that the Respondent failed to do the following: 1) monitor and submit monthly reports; 2) revise the sample site plan in accordance with a Level 2 assessment for the RTCR; 3) retain a certified operator necessary for DBPR treatment technique; 4) provide Public Notice of violations; and 5) submit CCRs. Therefore, EPA is now ordering the Respondent to address the violations and comply with the SDWA regulations.
