On or about September I I - 14, 2018, representatives of EPA conducted an inspection of the Respondent's facility. EPA identified the following violations of RMP 40 CFR Part 68 rules: Failure to Update Process Safety Information, 68.75(d); Failure to Timely Conduct Refresher Training, 68.71(b) and (c); Failure to Timely Conduct Training Required by Management of Change, 68.75(c); and, Inadequate Emergency Response Program, 68.95(a).
