Case summary
On March 3, 2015, EPA Region 6, issued the Final Order for the Proposed Administrative Order (PAO) under its authority pursuant to Section 1423 of the Safe Drinking Water Act to Smith Petroleum, LLC, located in Barnsdall, Oklahoma. The PAO was issued in response to the following violations : (1) failure to conduct a mechanical integrity test and (2) failure to maintaining an injection well in a manner which could allow fluids to move through the well bore into underground sources of drinking water (USDWs). The PAO requires Smith Petroleum, LLC, to either (1) conduct a mechanical integrity test; or (2) submit plans for the future use of the wells to the EPA, including proposed procedures to prevent contamination of USDWs. Smith Petroleum, LLC, must respond to EPA Region 6 within thirty days.
