The AO was issued in response to the following permit violations: 1) Operation and Maintainance requirements; 2) Record Keeping requirements; 3) effluent limit violations; and 4) failure to submit quarterly reports. The AO requires the Village of Chama to 1) provide maintenance to equipment to ensure effective treatment of the wastewater; 2) submit a Nutrient Managment Plan and compliance summaries; and 3) certify compliance with permit limitations. The Village of Chama must respond to EPA Region 6 within thirty (30) days of receipt of the order.
