Case summary

On August 27, 2012, EPA Region 6 issued a Consent Agreement and Final Order (CAFO) under its authority pursuant to Section 1423 of the Safe Drinking Water Act to Osage Energy Resources, LLC, located in Pawhuska, Oklahoma. The CAFO was issued in response to violations of: 1) failure to demonstrate mechanical integrity of two injection wells; and 2) operating one injection well without authorization to inject. The CAFO requires Osage Energy Resources, LLC, to pay an administrative penalty. Osage Energy Resources, LLC, must respond to EPA Region 6 within thirty (30) days of receipt of the CAFO.
