On July 22, 2011, EPA Region 6 issued a Proposed Administrative Order (AO) under our authority pursuant to Section 1421 of the Safe Drinking Water Act to Jerry L. Clemishire located in Hominy, Oklahoma. The AO was issued in response to violations of maintaining an injection well in a manner which could allow contaminants to move into fresh groundwater resources. The AO requires Jerry L. Clemishire to complete appropriate corrective actions on the well to prevent contaminants from moving through the well into fresh groundwater resources. Jerry L.
