Case summary

SFI of Arkansas LLC. On or about March 11, 2011, a desk audit was initiated on SFI of Arkansas pursuant to HQ’s Data Quality Initiative. SFI had supplied an incorrect NAIC’s code on their 2009 Form R’s. During the course of the investigation it was initially discovered that SFI had allegedly failed to submit Form R reports for the previous calendar years, 2005 through 2008. On July 19, 2011, a concurrent Complaint and Consent Agreement was issued to SFI of Arkansas. The facility has agreed to pay penalty. Penalty paid 8/24/2011. Case Closed.
