Violations: 1. Part 4.2.7.2.1.5 of the Permit was violated in that the required facility inspections were not be conducted. 2. Part 4.4 of the Permit was violated in that the non-storm water discharges were not certified and identified in the Storm Water Pollution Prevention Plan (SWPPP). 3. Part 4.2.7.2.2.1 of the Permit was violated in that the SWPPP does not describe sediment controls and Best Management Practices (BMP's) that are needed to address areas that are subject to erosion and are not stabilized. 4. Part 4.5 of the Permit was violated in that the SWPPP did not include either supporting documentation or a certification indicating that permit eligibility requirements had been met with regard to threatened and endangered species.
