Case summary
VIOLATIONS: Violated Osage underground injection control (UIC) program regulations by failing to comply with permit requirements to submit a corrective action plan if he static fluid level in an injection well is less than 50 feet below the base of underground sources of drinking water. Also violated UIC regulations and the injection well permit by maintaining the well in a manner which could allow fluids to migrate into underground sources of drinking water. REMEDY: Complete corrective actions by November 1, 2004. Corrective actions may be to plug well, demonstrate its mechanical integrity, or any other action approved by the EPA. PENALTY: None
