Case summary
VIOLATIONS: Failed to assure that fluids could not move through a well bore and contaminate an underground sources of drinking water (USDWs) by maintaining an injection well with a static fluid level in the well annulus above the base of USDWs. Also, failed to plug that well within one year after termination of injection operations. Failed to submit an annual operations report for one well for the period January through December 2003 REMEDY: For the well with a static fluid level above the base of underground sources of drinking water, either complete plugging or submit a plan for future use, including a plan for assuring that fluids will not move through the well bore into underground sources of drinking water, by September 30, 2004. Appropriate corrective action may be to demonstrate that the well has mechanical integrity, reduce the annulus static fluid level to at least 550 feet subsurface, or any other action that would prevent fluid movement into USDWs For the other well, submit the required annual operations report within 30 days after the effective date of the order. PENALTY: None
