VIOLATIONS: Violated Safe Drinking Water Act requirements by: a. Maintaining five injection wells in a manner that could allow fluid movement through the well bore into underground sources of drinking water. b. Failing to submit proposed plugging procedures to the EPA before initiating plugging of one injection well. c. Failing to submit a report of completed plugging operations to the Bureau of Indian Affairs. d. Failing to comply with compliance terms of two administrative orders. RELIEF: Replug one injection well to assure that plugging is compliant with Osage underground injection control program requirements. Complete appropriate actions on five injection wells to assure that fluids will not move through the well bores into underground sources of drinking water. Complete work no later than October 1, 2004/ PENALTY: Up to $125,000
