VIOLATIONS: Failed to prevent fluid movement into underground sources of drinking water (USDWs) by maintaining an injection well with a static fluid level in the well annulus above the base of USDWs. REMEDY: Complete corrective action on the well be September 30, 2004. Appropriate corrective action may be to plug the well, demonstrate that the well has mechanical integrity, reduce the annulus static fluid level to at least 560 feet subsurface, or any other action that would prevent fluid movement into USDWs PENALTY: None
