Case summary
VIOLATIONS: Violated Osage underground injection control (UIC) program regulations by maintaining one injection well in a manner that could allow fluid movement into underground sources of drinking water and by failing to plug two injection wells within one year after termination of injection operations. REMEDY: Plug one injection well on or before August 1, 2004, unless the Bureau of Indian Affairs assigns the well to another owner/operator. Either complete appropriate actions on one well to protect underground sources of drinking water by August 1, 2004, and submit a plan for future use or plug the well according to Osage underground injection control regulations. PENALTY: None
