Case summary
VIOLATIONS: Violate Osage underground injection control program regulations by failing to plug one injection well within one year after termination of injection operations. Also, violated the regulations by maintaining the injection well in a manner that could allow fluid movement into underground sources of drinking water. REMEDY: Either plug well according to Osage underground injection control regulations, demonstrate that the well has mechanical integrity, or complete other corrective action to be approved by the EPA by July 1, 2004. If corrective action, other than plugging the well is completed, must submit an acceptable plan for future use of the well to the EPA. PENALTY: None
