KRespondent operates injection wells in Cotton County, Oklahoma as provided by mineral mining leases. Respondent is subject to regulations under the Safe Drinking Water Act, Underground Injection Control (UIC)Program. Applicable UIC regulations require that Respondent submit an annual report of well operations which includes average and maximum monthly injection pressure and volume of fluids injected each month. The regulations also require that injection wells be plugged and abandoned within two years after termination of injection operations. Respondent failed to submit the required reports for one injection well for the period January through December 2001. Also, respondent failed to plug four injection wells within two years after termination of injection operations. The AO required Respondent to submit the required report and to plug the four wells.
