'VIOLATIONS: 1) Certified DMRs filed by the permittee show discharges of pollutants from the facility that exceed the permitted effluent limitations; 2) Unauthorized sanitary sewer overflows; 3) Facility inspection violations; and 4) Discharge Monitoring Report violations. **RELIEF: 1) Within thirty days, the Permittee shall submit a written report detailing the specific actions taken to correct the violations and prevent recurrence of the violations; 2) If complete correction and/or elimination of the violations is not possible within thirty days, submit a written comprehensive plan to do so; and 3) Within forty-five days, arrange a Show Cause meeting with EPA.
