'**VIOLATION: 1) Discharging pollutants from a point source to waters of the U.S. without a permit. **RELIEF SOUGHT: A) Immediately take measures to prevent any discharge of pollutants to waters of the U.S. and within 30 days, demonstrate proper design, construction, operation, and maintainance; B) Within 30 days, initiate a weekly visual inspection program to determine if any discharges are occurring and notify EPA by phone within 24 hours and in 5 days in writing if a discharge has occurred; C) Within 30 days, submit information concerning any discharges of pollutants from the facility to waters of the U.S. within the last five years; and D) Submit a Notice of Intent (NOI) to be covered under the EPA CAFO general permit.
