'**VIOLATION: 1) Discharges of of pollutants to waters of the U.S., from a point or outfall other than one authorized by the permit. * *RELIEF SOUGHT: a) Accomplish the tasks and comply with the schedule of activities contained in the ao; b) Start and completion of an activity shall be no later than the last day of the month and notification to epa of completion or start of an activity will be submitted within 15 days of start or completion; and c) Submit semi-annual progress reports on all activities on every Jan. 15 and Jul. 15, until work required by the ao is completed.
