'VIOLATION: 1) Discharging pollutants from its facility which exceeded permitted effluent limitations. RELIEF SOUGHT: a) Within 30 days, conduct and submita written report for the following activities: 1) Provide certification that authorized agent had read and understands the npdes permit requirements; 2) Develop an in-house procedure to properly report bypasses and noncompliance as required; 3) Develop an in-house DMR submittal procedure; 4) Repair the gap in the side of the weir; and 5) provide written explanation for failure to submit completed DMRS for Nov. 2001 through Apr. 2002; and b) Within 60 days, conduct and submit a written report for the following activities:1) Develop a Preventive Maintenance Plan for the collection system; 2) Develop an Emergency Response Plan to address Bypassing; 3) Develop a Continuous Training Education Plan; 4) Submit verification that licenses are obtained by appropriate employees; and 5) Provide justification for delay in start of Phase II and progress reports on status of construction activities.
