''VIOLATIONS: 1) Maintaining injection well in a manner that could allow fluid movement into underground sources of drinking water; 2) Failure to plug and abandon an injection well within one year after termination of injection operations; and 3) Failure to submit annual operation reports. **RELIEF SOUGHT: a) Submit required annual operation reports for the well for the period April 1999 through March 2002 and corrected annual operation reports for the well for the period Aprill 1999 through March 2002 within 30 days; and b) Either plug the well according to regulations or submit a viable plan for future use of the well and complete corrective actions necessary to prevent fluid movement through the well bore into USDWS within 30 days
